With our Kit Program your practice will have the convenience of giving a newly diagnosed diabetic patient the tools they need to begin immediate testing compliance. The meter kit of your choice comes in our insulated cooler bag and includes meter, 10 strips, 10 lancets, lancing device, control solution, meter case, and instructions for use. This program was developed with the patient's health in mind. They acquire everything they need the day of their appointment so that they can not only begin testing immediately but also they can be properly instructed on how to use their new meter. Inside the front pouch of the kit bag will be a referral form. All that we ask is that you fill out the information requested and fax it in so that we may begin the enrollment process for your patient. Once form is completed by patient and doctor, it can be faxed and/or mailed to us and when we receive it we will follow up with patient within 1-2 business days. There is no cost to your office or your patient for these kits. We offer Abbott, Bayer, Roche, Lifescan, and many other meters to fit the needs for your office.
